You can download the W9 form, fill it out, and attach it to the email. The IC Agreement comes directly to us once it's signed, so there's no need to attach it. If you're not comfortable filling out a W9 form, you can wait until you've gotten your first driver scheduled. Please include the following information in your email to us:
